We run the second option that Hunky Bill mentioned, the app tracks usage of the app and if they exceed each plan limit it requests another recurring charge for that range for the future. It is far less intrusive to app users and guarantees a monthly payment. We also find stores are consistent or only ever increase so it works well, its rare for a store to start using or benefiting from an app less so we just handle these manually by request.
