I have created an app which does this (adding a free gift once a certain value of products has been added to the cart). It's a much simpler solution to the combination of apps Jordan came up with. I'm looking to make it as easy to use as possible.
Eventually it will be a paid app, but I would be happy to give free access to shop owners who can help me in with the beta testing (even once it's released) by testing and suggesting features.
