A little more involved than our little app https://apps.shopify.com/review-companion and I think we have advantage of sending the user to the product page and auto scrolling to the review segment and opening it ready for them to place the review thus making the likelihood of a review much more likely ;-)
Review Companion is due for some great updates too in the near future so watch this space
